Letter to change maternity leave start date

Use this letter to formally notify your employer that you wish to change the start date of your maternity leave. You must give your employer at least 28 days' notice before the new start date (if you are bringing it forward) or 28 days before the original start date (if you want to push the start date back. Please note that you cannot use this document if you are not an employee (i.e. you are a worker or self-employed).
