COP 8 - Application relating to EPA registration

This form can be used either to object to the registration of an enduring power of attorney (EPA) or to seek registration if registration has been suspended. It is only suitable for the donor of the EPA, an intended attorney or a relative of the donor who is entitled by the Mental Capacity Act 2005 to receive notice. If none of these terms describes you, you can still object but must use COP 1 rather than this form. This application must be filed within 5 weeks from the date when you first received notice of the application to register.
